First of all we have to identify Jesus as the Shepherd of the sheep.  This actually means He came by fulfilled prophecy going all the way back to Genesis 3 where Jesus is the Seed of the woman etc. Then we have to ask who this doorkeeper is which I believe is the Holy spirit who is the real author of the Old Testament prophesies. By our reading of the Old Testament we can identify Jesus as the Shepherd of the sheep.
Next Jesus identifies us as His own sheep and goes before them and they follow Him.  Jesus says they follow Him and not strangers. It makes me wonder when I see someone walking off from Christ, whose voice are they following?  However it may be a lack of knowledge of the word of God.  Jesus said in Matthew 4:4, "Man shall not live by bread alone but by every word that proceeds from the mouth of God." Neglect of the word of God can starve the soul.
Then there is finding pasture by going in and out.  I believe Jesus meant the Old Testament Word of God prophetically.  It could  be more.
